Supporting People in the Community: Self-Direction
The need for a New Model of Residential Support
Over the past 20 years, Advocates has worked closely with hundreds of self-advocates and their families, empowering them to self-direct their supports and meet their need for day services. As parents and caregivers age, we have heard consistent feedback about the need for a new residential model for adults with intellectual and developmental disabilities who wish to remain in their own homes in the community but require overnight support, backup staffing, and medical assistance.
In response, and in partnership with the people and families we support, Advocates has developed a proposed Agency Supported Residential Service model. Designed to bridge the gap and make self-direction a sustainable long-term solution, this model addresses the key concern that every caregiver of a loved one with an intellectual or developmental disability faces: “What happens when I’m gone?”
Working with the people we support, their families, our state partners, and other service provider organizations, we aim to help create a sustainable, non-certified residential option that values both independence and security while providing peace of mind for aging caregivers.

Self-Directed Services Currently Available
New York State offers people with intellectual and developmental disabilities the opportunity to choose a combination of supports and services based on their needs and preferences. Self-Direction provides you and your family with the flexibility to choose and change your goals, schedule, activities and staff based upon your wants and needs.
Self-Direction begins with a person-centered plan that identifies your specific goals and objectives. This is an individualized approach to planning that focuses on your strengths and capabilities while addressing your needs. We partner with you and your Circle of Support, including your Care Manager, to develop a comprehensive plan identifying the staff support, transportation, financial subsidies, and other services needed to help you live the life you want.
Identifying Your Goals
Everyone has the right to choose whether they want to have full control over managing their budget and staff within a pure Self-Direction model, or to choose more support from Advocates through our Agency Supported Community Habilitation service. Within both services, Advocates works with you and your family to develop and manage your customized plan on an ongoing basis. It is a collaborative model whereby you, your family and your Circle of Support work as a team with Advocates to hire, train, and supervise your staff.
Since 2010, Advocates has been Central New York’s leading provider of self-directed services. Advocates differs from other providers with our ratio of employees to the number of people we support with 1:1 Mentor services. Through the commitment of over 1,400 dedicated employees, Advocates proudly supports over 1,000 people with intellectual and developmental disabilities and their families.
